The URL will be something similar to this. arthuronlineapp(dot)com/formbuilder/endpoints/{rand}/{rand}/{rand}.html?t=arthuronline&l=en&a=formbuilder&b={form_id} Replace {rand} with one of these three letters. i = input field type ii = hidden field type iii = output field type You will then replace {rand} with an integer starting from 1 going up until 9999 which represents the ID number for the form field Then replace {rand} with a string representing the label for that particular field within the form Finally replace {rand} with a whpe number between 0 and 100 representing the percentage value of the field Now click on Public Data Fields (see image below. This will take you to a page where you can choose what kind of data fields you want included in your form builder app form including email address fields, phone number fields, date picker fields, dynamic text fields and select dropdowns among others (see image below. In addition to choosing the data fields you want included in your form builder app form, you can also choose whether or not to include items like required validation messages and success messages in your form builder app form (see image below. You can now publish your form by clicking on Publish Form (see image below. Your form page will now look something similar to this (see image below.
